HOW TO CLAIM
The COVID cash back benefit, reimburses amounts utilized following hospitalization from COVID -19 up to the cover limit. The claim form should be properly filled, signed and stamped by the treating doctor.
The completed claim form should then be sent to us with original bills, receipts and discharge summary.
